This case highlights the treatment of a 70-year-old patient with significant bone loss in the upper right posterior jaw. Because there was insufficient bone to support dental implants, the area was first carefully reconstructed to create a stable foundation for implant placement.

This case involved teeth #2 and #3 in the upper posterior jaw, which were determined to be hopeless and required extraction. Following extraction, the sites underwent socket augmentation; however, subsequent CBCT imaging showed that the remaining bone beneath the maxillary sinus was still extremely limited—approximately 2 mm in areas. Implant planning demonstrated that there was not enough vertical bone to predictably support implant placement without first rebuilding the site.
Because of the significant bone deficiency, a lateral window sinus augmentation was performed to create sufficient bone height for future implants. After healing and development of the grafted bone, implants were planned for the #2 and #3 positions using 3D CBCT imaging and subsequently placed with guided implant surgery for precise positioning.
Following approximately nine months of healing and integration, the implants were restored with crowns. The progression shown in these images—from extraction and severe bone deficiency through sinus augmentation, guided implant placement, and the final restorations—demonstrates how advanced bone-regeneration techniques can make implant treatment possible even when very little native bone remains.
